Children's Health Home of Upstate NY
The Children’s Health Homes of Upstate New York, LLC (CHHUNY) was established to plan for and operate as a Health Home for Children. CHHUNY is using a model of care management based upon the belief that every family has strengths and that families are the natural and most effective drivers of their care. This model also ensures that families have ‘‘voice and choice,’’ at every stage in the care management process. CHHUNY is accepting referrals for enrollment of eligible children/youth into Health Home Care Management Services.

Early Childhood Investigations
Early Childhood Investigations offers free webinars to early childhood educators. The series is produced by Engagement Strategies, LLC., and presented by leading authors, experts, and leaders. Now, you can participate in conference-quality training without leaving your home, classroom, or office.
Pediatric Steps to Literacy
The Pediatric Steps to Literacy, One Book at a Time project, funded by the Mother Cabrini Health Foundation with additional support from Howard P. Milstein, aims to ensure that children in at-risk communities develop proficient literacy skills and become lifelong learners poised for success.
Project Teach
Project TEACH is a New York State initiative aimed at enhancing the capacity of maternal and pediatric primary care providers to address mild-to-moderate mental health concerns in children and families. The program offers valuable resources, including telephone consultations, direct consultations for selected pediatric patients, CME-certified training, and referrals to mental health professionals and community resources. By supporting providers, Project TEACH ensures that children and families receive timely and effective mental health care.

Reach out & Read
This unique initiative partners with healthcare providers to encourage daily family reading from an early age. By integrating reading into pediatric care, Reach Out and Read aims to level the educational playing field, ensuring that income, race, and location do not determine a child's potential for success in kindergarten and beyond.
